ZP120 is an “opioid receptor like 1” (ORL-1) receptor agonist, a novel compound that incorporates Zealand Pharma’s SIP®-technology. The compound produces dilation of peripheral blood vessels and lowers blood pressure. Peripheral sympathetic nerves innervate blood vessels and are responsible for stress-induced increases in blood pressure. In response to stimulation, the sympathetic nerves release the neurotransmitter norepinephrine from nerve endings, which in turn is responsible for nerve-mediated constriction of the peripheral blood vessels and increased blood pressure. Conversely, reduced release of the neurotransmitter norepinephrine is associated with dilation of blood vessels and reduction in blood pressure. We have demonstrated that ZP120 has the unique pharmacological property to inhibit norepinephrine release from peripheral nerves in the blood vessels and this mechanism is responsible for the blood pressure lowering effect of ZP120. In contrast to conventional blood pressure lowering drugs, lowering of blood pressure with ZP120 does not elicit reflex increases in heart rate. From these pharmacological observations, we expect that the blood pressure lowering efficacy of ZP120 is most pronounced during conditions with elevated sympathetic nerve activity (i.e., high stress conditions) and that it can be used safely in patients with normal sympathetic nerve activity.
